To ensure we provide best service please state below your preferred days and times for us to contact you and allocate future appointments.
By submitting this form, you agree to:
- Failure to attend or short notice cancel an appointment can result in a “Flag” being applied to your records. As an NHS patient this must not exceed two instances within a one year period as this would result in your profile being inactivated.
- If a new NHS patient misses or short notice cancels their first appointment, the practice will not provide another appointment and may need to go back to the waiting list.
- NHS patients must maintain visits based on their recall frequency and should ensure they are seen for an examination at least once within a two year period. Failure to do so may result in removal as an NHS patient so the practice can offer this to someone on our waiting list.
